It seems like one of the two release buttons is missing the part that sticks out, thus requiring a tool to push in. Makes adjusting the headrest very hard.

but both my headrests are like this, so is it just a terrible, but intentional, design?

Yes it's the design. It is difficult but the button that is recessed is the one on my truck that controls the movement and I have gotten the hang how to use it. With kids car seats in and out with the tip restraint I move my headrest on a weekly basis

yes, I have this problem on a daily basis adjusting car seats, too... that's what I was doing just now when I snapped this pic

See manual. By design. Headrests are not adjustable. They can be removed but apparently should only be done by service. In the manual, they also tell you to strap a child seat’s rear anchor strap over the top of the headrest instead of through the sliver of an opening.

Most folks who have child seats will either just click the tabs in to move the headrest up or remove it altogether. Thought it was an asinine design but apparently it’s to meet safety standards.

I had no clue the anchor strap was to go over top of the headrest. Seems like it would compress and crush it once you tightened.

I had no idea it was supposed to go over the headrest (my bad for not having noticed that in the manual, which I did, in fact, at least attempt to read). I'm going to go experiment with compressibility of both the seatback and the headrest now.

I wonder if the answer, which is something else I've wanted anyway, is a curved rigid "shield" that has two holes that allow the headrest to pin it in place. That could provide a rigid surface for the latch strap to travel across, but it could also create some mount points for other things like my seat covers. I'll demonstrate with a piece of paper and post a picture later.
