Tesla FSD fails to see road debris, Model Y gets airtime

I have a model Y with FSD. Several times recently I have had to rapidly take control when it saw old tire skid marks in the lane ahead and took violent evasive action. With other traffic around, the car’s maneuvers to avoid an imaginary obstacle were very dangerous. It is more stressful to use FSD than to drive myself. Still, I turn it on occasionally just to see how it does, under very close supervision.
What version FSD? and does your vehicle have HW2,3 or 4 hardware? Currently most owners can driver for hundreds of hours and never touch the wheel or pedal a single time. Even their robotoxi fleet has done loads and loads of miles between any minor interventions. As per Elon's X post today FSD Version 14 is dropping next week and then another the week after. It's said to be levels of magnitude better than the current public version13 even. the version the robotaxis are using.

What Model and year is your Tesla Docwhiz? What is the FSD version? and are you running on hardware HW3 or 4? Just want to make sure you are someone who has experience using FSD daily to see the progression over the past months and years and can truthfully vouch for the fact that Tesla FSD hasn't had any significant improvement?
Because almost everybody else (who owns a Tesla with FSD) has seen leaps and bounds. Heck; from even a few years ago with many complaints of phantom braking and such, we are now at a government approved Tesla self driving robotaxi network in operation in Austin.

I don't use AP or FSD much primarily because of phantom braking. Also random erratic behavior.
I turn AP on occasionally when I am the only one in the car but find it fails within a few minutes.

I turn AP on occasionally when I am the only one in the car but find it fails within a few minutes.
Try a newer one with HW4 and FSD ver13 (next week Ver14). You'll see a major improvement. HW3 can only get 12.6 max.
