Even wear aside, a 5-wheel rotation should theoretically get you 20% more life out of the tires. The spare will wear out over time even if unused. Might as well get the life out of it.

Yeah, still relevant. Things like ABS, traction control, and torque vectoring all rely on the wheel speed sensors...or I guess in this case just the motors themselves. If they aren't all at roughly the same speed in a straight line then it'll likely assume a wheel is locking up or spinning. So mechanically there won't be an issue, but the truck may do weird stuff. When I first started racing Corvettes I had a few events people call "ice mode" because the stagger front to back wasn't close enough to the 1" in diameter that was spec'd. Basically the car decided that the wheel speed differential front to rear indicated a slick surface and it dramatically reduced brake pressure to the point you almost didn't have brakes. Kinda scary in a controlled environment...on the street I can't even imagine how bad it could get. Now that said, I'm talking about differences of 0.25-0.5" or more. You won't see that from normal wear on tires unless you go nuts and try to put a full-tread tire on with a nearly bald tire. Tough a never used spare could get you in that position at some point, I spose.
Yes, comes with jack and tools. Supposedly the thing the tools are mounted to doubles as a jack base to make it safer to use off-road. IIRC, there's a mode buried in the menus that you need to enable so you can lift the truck without the air suspension trying to level things as you lift.
